As part of its work to support health promotion efforts at the community level, the U.S. Centers for Disease Control and Prevention’s Division of Community Health (CDC/DCH) has awarded cooperative agreements to American Heart Association, American Planning Association, and the National WIC Association for sub-awards to chapters/affiliates in order to build and strengthen the community’s ability to use population-based strategies that create healthier communities. The 50 chapters/affiliates (which can be viewed here) will be working to reduce tobacco use and exposure, improve nutrition, increase physical activity, and improve access to chronic disease prevention, risk reduction, and management opportunities.
