This position provides nutrition services to prenatal, postpartum, breastfeeding women, infants, and children from birth to five years of age. The duties include conducting nutritional assessments, providing individualized and evidence-based nutrition counseling and education, developing care plans and goal setting, promoting, and supporting breastfeeding, and providing community referrals. Accurately documents assessments, care plans, notes, and correctly enters into the electronic medical record.
